Application 5S, A Spreadsheet linked with G. forms

This is for my college project, I made the functional part of the application and now I need help with the technical part.
I explained every thing in the file attached.
A spreadsheet and a google forms : Based on answers of the users in the google form, the spreadshhet is filled with data. There are automatic emails, and automatic updates based on several conditions that I need help with.
I already have the templates and the checklists done what I still need is the link between all of them.
