1 Questionnaire structure
1.1 Welcome page

The respondent shall identify himself. No controls will be made. The aim is that the questionnaire will be attached to the declared respondent

1.2 Question page
Questions will be grouped under relevant question groups
Answers shall be either « YES » or « No » or « Not applicable ». A comment field will be offered to the respondent in order to collect rationale that would substantiate his answer.

A weight (importance) shall be attached to each question.

1.3 Results page
2 Expected types of results:
1) an overall score
The questionnaire shall compute (add) the result of all questions according to their individual score (question_weight*answer).
According the final score , an appropriate recommendation text shall be displayed.

2) a radar representation
Each group shall be an axis of the radar representation.
Results shall be normalised on the 4 level scale.
3 Design
All formulas used to compute the results and to display them shall be copied onto a separated word/text document
All questions texts and assigned points shall be modified the BPA

4 Implementation
To questionnaire shall be developed onto the BPA LimeSurvey platform

5 Acceptance test
5.1 Administrator take over
A BPA administrator shall modify the text of each questions and modify the rating to be applied to each question.

5.2 User experience
A BPA user shall connect and respond to modified questions.
The result shall be-as expected
